Completely filled in and self attested print out of the Registration form which was submitted online.

Attested copy of the Birth certificate from Nagar panchayat/Nagar nigam/Gram Panchayat Secretary (Birth certificate issued online by the State govt. is acceptable, if it’s not hand written and legitimate.

Proof of Address of parent(s)(Adhar card/Driving License/Passport/Ration Card).

Latest Passport size Photographs of the child,father,mother and Guardian.

One recent Family photograph (Mother, Father and the child in one frame).

Aadhar card (s) of the child, Mother & Father.(compulsory)

Transfer Certificate of Previous School (having PEN number)

Parent ‘s ID card issued by Project authorities If working with UPRVUNL,OTPS OBRA

VERY IMPORTANT: PEN (Permanent Education Number) is mandatory for the the students of class I ,Onwards Without PEN Student's registration/admission will not be accepted
